You will get basic medical cover without it, but I’d get medical cover and should the worst happen, you would get (FOR EXAMPLE) air ambulance back to the UK for Intensive Care.

The quick answer is no. however you will get basic medical cover with the E111 form Which i think they now call the European Health Insurance Card. This will only cover you for hospital treatment. you may find it better to get a cheap 1 year travel insurance. it may cost 150€ or more but you will have no worries while in Spain.
