Definitely, the card only covers basic treatment and your insurance will be invalid if there is any existing medical condition that you did not declare

You most definitely need to let your insurance company know. If they ask and you dont tell, and something happens while you are driving they have every right to decline any claim that may occur due to non-disclosure of information. If they find out before an y claim they have a right to cancel you for non-disclosure. You dont want this to happen as you may not know this but you have an insurance history record, and when you get new insurance most places will look this up and if you have been cancelled before by an insurer this could cause your policy to be sent to a residual insurer, which is usually about 3x normal rates, and with my experience with people with medical conditions theres never a problem insuring them, we just want to make sure youre safe!!
