I am paying tax on permanent health insurance now being paid to me by my employer . My employer paid the premiums to the insurance company. I understand that this payment should be made to me free of tax.

This is a benefit of your employement and it is part of your remuneration package therefore tax charged on you under ITEPA 2003. Your employer will have notified HMRC of this benefit on a P11D return and and tax due on it will be taken into consideration through your tax code.
